About the Position
This is a weekend, weekday and holiday coverage PRN position to cover scheduled intake team members during their time off. The PRN QA/Intake Nurse will review referrals from various sources such as hospital portals, faxes, phone calls, and emails to determine home health eligibility. This position will coordinate with intake and billing teams to process referrals and input/upload all clinical records into electronic EMR. Capabilities of communicating with referring physicians, doctors’ offices, case managers and facilities. The PRN QA/Intake Nurse is responsible for performing QA of clinician documentation, chart audits for QAPI and compliance as directed. Position requires skills of a clinician with home health experience in order to clinically review home health documentation for eligibility and coding. Hospital/Facility discharge planning and Home Health knowledge is preferred.
Job Duties
· Effectively communicates with all physician’s offices and case managers to obtain needed documentation to process home health referrals.
· Understanding of PDGM coding to accurately assist in diagnosis identification for initial intake eligibility.
· Creates Face-to-Face documents with accurate PDGM code to complete home health referrals.
· Monitoring and assuring that agency is within Medicare guidelines with Face-to-Face requirements.
· Knowledge of authorization processes of commercial insurances to process complete home health referral.
· Inputs admission documentation in the EMR chart and assign the admission/ start of care.
· Perform warm welcome call with admitting patient to discuss home health process, verify start of care date, verify MD, and notify patient of benefits/coverage.
· Ability to complete tasks in Microsoft Office Suite including Word, Excel and One Note to completeness and with efficiency.
· Performs QA of clinical documentation of skilled nursing visits, HHA aide care plans, HHA visits and additional clinical documentation as requested.
· Performs audits of clinical documentation and charts to Medicare compliance.
· Maintains confidentiality of patient and staff information at all times.
· Attends mandatory meetings/in-services and maintains 100% compliance with mandatory data based in house training.
· Immediately reports abuse/suspected abuse to Administrator.
· Meets with Lead Intake Specialist and Director of Clinical Services regularly to discuss home health referral status, processes, and procedures.
· Participates in patient satisfaction calls and program calls as requested.
· Performs additional job duties as assigned.
Job Requirements
· Must possess the ability to deal tactfully and compassionately with patients, family members and caregivers
· Must be willing to and able to participate in a teamwork approach, working with all disciplines involved in the patients care/case
· Proficient in Microsoft Office, One Note and Excel, be willing to adapt to various EMR systems currently used as well as other programs used throughout the community
· Communication, documentation, and group dynamics skills
· Must be available to work weekends and some holidays
· Must be able to multitask and process multiple referrals daily
· Must be able to work with frequent interruptions
· Always maintain privacy and confidentiality of patient’s information
· Must possess sight/hearing, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met, to include use of departmental equipment and/or supplies
· Must function independently, have flexibility, personal integrity, and the ability to work effectively with the residents/patients, personnel, and support agencies
· Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times, hostile people within the community
· Must be able to read, write, and speak English in a proficient and understandable manner
· Must possess the ability to make independent decisions when circumstances warrant such action
· Must possess the ability to deal tactfully with personnel, patients, family members, visitors, government agencies/personnel and the general public
· Must be able to follow oral and written instructions
· Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to handle residents bases on whatever maturity level they are currently functioning
